Cancelation Policy

 

                                                                                JAG Fitness Policies

1. Abuse of equipment will not be tolerated

2. Equipment or weights must be replaced after each use.

3. Solicitation for sale of merchandise, supplements, drinks or unapproved personal training is not permitted on JAG Fitness premises.

4. Alcoholic beverages, smoking and illegal substances are not permitted on JAG Fitness premises.

5. Closed shoes and shirts must be worn at all times.

6. Loud or abusive language will not be tolerated.

7. Failure to comply with the above policies may result in cancellation of membership.

8. JAG Fitness is not responsible for lost or stolen goods.

9. All monies are NON-REFUNDABLE. 

 

LIABILITY WAIVER IN EXCHANGE FOR PERMISSION

In consideration of being permitted to utilize its facilities and equipment JAG Fitness (“The Club”), I understand and agree that neither JAG Fitness, nor its affiliates, owners, operators, agents or instructors may be held liable in any way for any occurrence in connection with my use of such club which may result in injury, death or other damages to me or my family, heirs, or assigns. I am cognizant of all the inherent dangers of exercise and the use of fitness and capability to the club safely. I understand that a physician should be consulted before commencement of any exercise program. I understand and agree that the terms herein are contractual and not a mere recital, and sign this document of my own free will. I will use the club only in the manner and for the purpose for which the club is designed and shall be responsible for damages to the club or other members caused by me. JAG Fitness is not responsible for any loss or damage, including theft, to property of its members or guests of members. I hereby agree to be responsible for any and all damages to the club, and I have read, understand and agree to Liability Waiver above.

 

I GIVE JAG FITNESS PERMISSION TO AUTO DRAFT MY ACCOUNT EVERY MONTH UNTIL I CANCEL MY MEMBERSHIP.

I AGREE TO GIVE 30 DAYS NOTICE TO DESK STAFF to cancel my membership OR I WILL BE CHARGED AGAIN.

I AGREE TO KEEP MY MEMBERSHIP FOR AT LEAST 4 MONTHS.
